Fixed Braces

These braces work with more intense forces and offer in a relatively short time aesthetic results.
For they are only applied to permanent teeth, only adolescents or adults are treated with this. Removable appliances would not lead to the desired result here.

“Inclined Plane”

This is a acrylate bloc that is used for the correction of crossbites. This one is applied to the opposing row of teeth with a special cement. Through the occlusal force a correction of the teeth into the right direction already ensues after 4-5 weeks. At this point the so called place holders should be mentioned. If children lose their milk molars too early, the place of the remaining teeth narrows. The teeth lying behind the gap fast move forward and prevent the proper breakthrough of the remaining tooth. As a counter measure a metal band is applied to the first permanent molars (6th) on which a wire is welded and which keeps the place of the milk tooth free and thus the unhindered breakthrough of the permanent tooth is possible.

An especially fast group of the fixed braces is the one of the “Rapid Expanders”

The so called hyrax is one that is spread the widest. A fast and far-reaching expansion of the upper jaw can be achieved with this. It is mostly used for adolescents, since adults already have an ossified bone sutures at the palate and thus only a combination is possible with the aid of a surgical intervention. At doing this, metal bands are applied to the molars of the upper jaw which meet in a screw at the palate. At the beginning the screw has to be activated each day to lead to the opening of the suture being located at the palate.

The effect is very spectacular: The two incisor teeth slowly diverge from each other, which is the secure evidence for the correct function. The emerged gap however closes very fast again on its own.

The largest group of the fixed braces is the Multi band apparatus.

Multi Band

Braces like this consist of retaining elements and actively working metal arches. Retaining elements: Bands that are cemented to the molars; brackets that are applied to the teeth with a special adhesive method (direct bonding). They are conceived as little locks which retain the active arches at the teeth. Brackets are made out of various materials: metal, synthetic, porcelain, etc.
In the bracket slots are the active metal arches that are also made out of different materials. These are either applied to the brackets with wire, gum or an own closing mechanism.

A lot of techniques of treatment have developed with Multi Band Apparatus. The brackets have changed a lot in form and material. Own bracket locks have been developed which enclose the active arches. Also the materials used for the arches have developed further. Previously simple steel arches were used, which have lead to the desired tooth movement through adequate bends. Today so called thermoplastic arches are on the market which regain their previous form at mouth temperature and thus exert the right force on the teeth autonomously. Especially at the beginning of a treatment they are very considerable because even teeth that are difficult to reach are easy to get in line due to its flexibility. Generally the Multi Band treatment lasts app. 18-36 months, dependent on the displacement. During the treatment the dental hygiene especially has to be focused. Due to the aggravated cleaning it can easier lead to carious lesions.
